Paperwork deadlines for fall sports 2017-2018:
For the classes of 2018, 2019 & 2020: Fall sports physicals and consent forms are due in the health office no later than Monday, June 12, 2017; Cheerleading is due June 2. If you do not have a complete and cleared packet including online consents, you will not be medically eligible to participate for the fall season. In addition, NJSIAA regulations require a 30 credit minimum earned from the current (16-17) school year to be academically eligible to participate in fall sports.
For the incoming class of 2012 and new students: Fall sports consents will be due on July 6, 2017; physicals are due no later than August 2.
If you have had an exam within 365 days of the first day of practice, you only need to provide a health history update. If not, a complete physical exam is required. For example; if you had your last exam on August 10, 2016 and practice starts on August 9, 2017, you only need to submit a health history update. If you had your last exam on or prior to August 9, 2016, you will need to submit a complete physical packet. This is true for all seasons so please make sure you have a valid physical exam date before the start of each season. Only the forms provided in the links below will be accepted as they are state regulated. We cannot accept your physician's office records/reports

Please read the directions for sports physicals each season as there are updates and new information regarding sports physicals.
Please be mindful of the deadlines for submissions for sports physical paperwork to the Health Office. Submission of the forms to the Health Office does not complete the process. Consents must be signed/accepted and the school physician must review and accept the physical forms and health history updates before an athlete is cleared for participation. Sports physical packets received after the due date are in jeopardy of not being cleared for participation in time for opening day.
Beginning this season, parents and athletes will be able to check for participation clearance through the parent portal. Log into parent portal and look for a flag in the area below the student's picture (it looks like a soccer ball). The flag will state "Medically Cleared ". Students without this flag are not yet cleared.
Parent emails of clearance will no longer be sent. Students can check for clearance as well through their Genesis grade book access.
